Category D

This form is submitted annually for activities that take place with students during the contract day. Approval will need to take place prior to the start of your group.

Please reference the Information Packet for activities that count for Category D.

Once your Category D is submitted, you will receive an email from PMI letting you know if its been approved.

Please keep student rosters of your activity meeting times. You will add them to the PMI Submission Form.

Step 1: Record Sheet

These are your PMI Record Sheets for the 2024-2025 school year.

After completing the document, you will upload it into the PMI Submission Form to complete your PMI.

Fall: Submission Form deadline is December 31st by 11:59 pm.

Spring: Submission Form deadline is the second Friday in May by 11:59 pm.

The link below will force you to make a copy. First, change the title of the document to your First and Last name PMI Record Sheet (December 2024), then change the share settings as follows:

  • Click on Share in the top right corner
  • Type in the add people field
  • Choose Commenter
  • Uncheck the Notify People box.
  • Click Share

Step 2: Submission Form

This is a required step!

The purpose of this form is to collect on the necessary information for HR and payroll. After you have completed your PMI Record Sheet, fill out this form to submit your PMI.

Upload your PMI Record Sheet

  • At the end of the form, you will be asked to upload your PMI Record Sheet. Select the Google Drive icon.
  • Locate your record sheet in your Google Drive. It should be called First Last name PMI Record Sheet (December 2024). If you didn't change the name of the document is will be called Copy of PMI Record Sheet (December 2024)
  • Upload the record sheet into the Google Form as a Google Sheet. (Please do not download it as a PDF.)
  • Please double check that the share settings are shared using the directions in Step 1.

PMI Submission form • Required •

Submission Form open December 1–31 and May 1–16.

Submission of hours does not guarantee approval. You will be notified by the committee once your accepted hours have been approved.

Important Dates

2024-25 School Year

Category D needs to be submitted for approval prior to the start of the activity.

Fall PMI Submission Form is due on or before December 31, 2024 at 11:59 pm.

Spring PMI Submission Form is due on or before May 9, 2025 at 11:59 pm.

Late submissions will NOT be accepted
